All Starz Gymnastics Sign-Up Sheet:

1. Select the class day and time from the schedule. Please include 1^st and 2^nd choice, classes do fill

quickly. Please fill out a liability form if you are a new student!

2. Calculate the amount due: Include the $30 Annual Family Registration Fee if due. Calculate a 15% family discount if applicable. Please pay full amount for your first child and

15% discount for each additional child.

*Split payment option is available. You pay the registration fee with half of the tuition at sign –up.

The second half of tuition will be due by noted date.

3. Return forms and payment to All Starz. Form of payment accepted (cash or check only.) Bring them into 14120 Zion Rd Thornville or mail them to P.O. Box 97 Thornville, OH 43076.

If you are enrolled in your 1^st choice you will not be notified.

If you DO NOT get into your 1^st choice we WILL contact you. If you are unable to attend your second choice you will receive a full refund. SORRY, no registration taken over the phone.

*Boys can be part of any class, but we do offer a few boys only classes to better train the boys due to the different gymnastics events that boys do. All Parent Tot and Tumble Tot 1 classes are combined with boys and girls!

Pre-Team & Select classes(coach invite)

All pre-team and coach select classes will set after team tryouts on June 6^th.

All 30 – 45 Minute classes will be upstairs at our new facility. There may be times the class instructor will take these classes downstairs.
